周仓是傩堂戏正戏《关爷斩蔡阳》中的角色，作为关羽手下的一员将领，负责探听敌情、与敌叫阵、传送消息等。剧中关羽先派周仓与蔡阳大战，周仓败北而回，只好又由关羽亲自上阵，与蔡阳大将大战数十回合。不料关羽也是败走，此时周仓献计请黄忠、张飞等人助战。后经大战，打败蔡阳得其首级，由周仓示于张飞，关羽才重获张飞信任得以进城。 剧中周仓面具绿底花脸，显得有几分怪诞，而他作为一员将领，能够上阵杀敌必然带有戾气，圆瞪的大眼与略作夸张的獠牙正好显示了这一点。
